Peptides for Men Over 40: Protocol Guide

After 40, three things decline simultaneously: growth hormone output drops about 14% per decade, NAD+ levels fall by roughly half between 40-60, and recovery from training takes noticeably longer. These aren't separate problems. They're connected, and peptides can address each one.

Growth hormone: ipamorelin + tesamorelin

GH production peaks in your 20s and slides downhill from there. By 40, most men produce significantly less GH per pulse, and the pulses themselves become less frequent. The effects: slower recovery, harder time holding lean mass, more visceral fat, worse sleep quality.

[[Ipamorelin|17]] (GHRP, 200-300mcg 2-3x daily) restores GH pulse amplitude without touching cortisol or prolactin. [[Tesamorelin|18]] (GHRH analog, 1-2mg daily) adds the production signal. Together they recreate younger GH patterns. Tesamorelin has specific data on visceral fat reduction (15-18% decrease in Phase III trials) and cognitive improvement in older adults.

The GHRP+GHRH stack is the gold standard for age-related GH decline. Run 8-12 weeks, 4 off, repeat.

Metabolic slowdown: tirzepatide or AOD-9604

Metabolism slows with age. If weight is creeping up despite no changes in diet or training, [[Tirzepatide|10]] is the heavy hitter (22.5% weight loss at 15mg in SURMOUNT-1). For those who prefer targeted fat loss without appetite changes, [[AOD-9604|16]] at 250-500mcg/day directly mobilizes stored fat.

Cellular energy: NAD+

NAD+ decline hits everything: mitochondrial energy, DNA repair capacity, sirtuin function. [[NAD+|14]] or [[NAD+ Pen|35]] can restore levels. Animal studies show mitochondrial function reverting to young levels within a week of supplementation (Gomes et al., Cell, 2013). If fatigue is your constant companion despite adequate sleep and training, the NAD+ angle is worth exploring.

Joint and tendon maintenance

Connective tissue gets more brittle with age. BPC-157 ([[BPC-157|22]]) at 250mcg/day supports tendon and ligament repair. Many men over 40 run low-dose BPC-157 as ongoing joint maintenance alongside their training program.

Building a men's 40+ protocol

Foundation: Ipamorelin + Tesamorelin for GH (8-12 week cycles).

Add NAD+ for cellular energy (ongoing).

If weight is an issue: add Tirzepatide or AOD-9604.

For joint support: add BPC-157.

For longevity: add Epithalon ([[Epithalon|15]]) courses twice yearly.
